As part of our effort to protect the privacy of your personal information while visiting the Site, we created this web privacy statement to inform you of how we collect and use information that you provide to us via the Site and answers commonly asked questions regarding the privacy of your individual information.


What personal information does the Company collect?

We may collect information that is identifiable to you if you provide it to us when you visit the Site, including any applications made available to our customers and their users through the Site. Such information may include your name, address, email address, username, and password in addition to the general applicability of this Web Privacy Statement to those products and services, use of those applications and any information collected there through, are governed by specific end user agreements, including any business associate agreement that may govern the use and disclosure of individually identifiable health information you may provide through the Site. In the event of a conflict the terms of such end user agreements and any corresponding business associate agreements control with respect to use of data submitted to applications made available through the Site, including individually identifiable health information. Feedback or “contact us” forms used in Company correspondence specifically request that you not include personal information such as usernames, passwords, social security numbers, and private health information.


How does the Company protect the security of your information?

We use reasonable security measures designed to prevent unauthorized intrusion to the Site and the alteration, acquisition, or misuse of personal information. However, the security of information transmitted over the internet cannot be guaranteed. You agree we are not responsible for loss, corruption or unauthorized acquisition or misuse of personal information that you provide through the Site that is stored by us, or for any damages resulting from such loss, corruption or unauthorized acquisition or misuse.
